The CaseAlert software developed by MEDecision is to be used in the “Florida: A Healthy State” disease management program, a collaborative initiative between Pfizer and the state of Florida. The Healthy State initiative is designed to address the critical health care needs of chronically ill Medicaid recipients.
Under the Healthy State program, Pfizer will work with the state’s Agency for Health Care Administration to target four chronic medical conditions in the Medicaid population: heart failure, diabetes, asthma and hypertension. The hospital-based disease management program will provide onsite care managers to work with chronically ill patients on a regular basis.
CaseAlert software uses expert-based algorithms and predictive methodology to analyze provider, facility and pharmacy data to identify members who are, or are likely to become, high-risk and/or high-cost patients.
The software combines artificial intelligence technology and clinical expert opinion to enable health care planners to identify and stratify patients needing disease management that traditional methods miss.
